Everyone has unique goals and expectations for their recovery following injury, illness, or surgery. That is why our rehabilitation process starts by listening to patients’ concerns and priorities. Rehab @ Weiss, A Program of Schwab Hospital at Weiss Memorial Hospital focuses on the patient first, in both inpatient and outpatient settings.

Acute Care
Seeing patients early in their hospitalization may speed recovery and prevent additional complications from prolonged bedrest. Our therapists work to improve functional status, provide patient/family education and/or make recommeRehabndations for assistance or further care upon discharge. Visit Rehab Services, 1st Floor, Weiss Memorial Hospital.  Hours are Monday through Friday, 7:30 am - 4:00 pm and Saturday and Sunday, 8:00 am - 12:00 pm.

Inpatient Rehabilitation
Patients for whom intensive inpatient rehabilitation is appropriate, work with a patient care team to prepare for life after discharge by participating in individual interventions, group treatment, and community re-integration activities. Referrals for admissions are accepted from physicians, case managers, social workers and community agencies, or by self-referral. Our rehabilitation staff will work with the referral source to determine eligibility for the program and to arrange for admission. Visit the Rehab Unit 4NW, C Elevator 4th floor, Weiss Memorial Hospital.  Hours are Monday through Friday, 8:00 am - 4:30 pm and Saturday, 8:00 am - 2:00 pm.

Outpatient Rehabilitation
For patients who have left the hospital or who live in the community and have therapy needs, therapy services are also available on an outpatient basis. Our outpatient rehabilitation therapists have advanced competency in orthopaedic and neurologic treatment, vestibular rehab, hand therapy, pulmonary rehabilitation, and speech/swallowing services. Our outpatient team can help determine the appropriate site of care for you and assist with obtaining referrals and pre-certifications. Flexible hours are available for your convenience. Visit Rehab Services, 1st Floor, Weiss Memorial Hospital.  Hours are Monday through Friday, 7:30 am - 6:00 pm.

The World Health Organization defines health as "...a state of complete physical, mental and social well-being...". In order to address the many factors that contribute to each patient’s unique definition of well-being, professionals skilled in all aspects of rehabilitation staff the Rehabilitation Center. These professionals work together as teams who specialize in rehabilitation medicine. Team members may include:

  • Psychologists

In all settings, patients and their family members work together with our rehab teams to improve function, learn techniques to manage daily activities, and identify ways to return to meaningful living. Because we believe that education is key to effective rehab, our patients receive the following information as part of the rehab process:

  • Medical diagnosis and its implications for functional ability
  • Ways to compensate for impairments
  • Ways to prevent further impairment
  • Adaptation to changes in level of function
  • Strategies for an active, healthy lifestyle

Our program philosophy and progressive clinical practices help our patients achieve a key goal of the Healthy People 2010 initiative: to increase quality and years of healthy life. Our patients are assisted with regaining independence, pursuing active, healthy lifestyles and resuming participation in community life.
