A tremendous amount of information is generated by a hospital visit. As patients are diagnosed and treated at Weiss Memorial Hospital, the Health Information Management (HIM) department collects and stores information about each patient. The department is responsible for maintaining and protecting the medical records of all Weiss patients.
All medical records at Weiss are stored electronically. Such automation ensures timely retrieval of records for patient care and customer requests. The department provides release of information Monday through Friday, from 8:00 a.m. to 4:00 p.m. HIM releases medical records for patients who want copies of their records for further care or other reasons. The department follows all state and federal regulations for the release of health information.
Please use the following forms to request medical records. You may fax your completed form to (773) 564-5829:

The HIM department's other responsibilities include coding, transcription, deficiency analysis, physician record completion and electronic record processing. The coding section performs coding on all inpatient, emergency and outpatient services. The department employs only credentialed and experienced coders who are certified through the American Health Information Management Association, a nationally recognized association for medical record professionals. All transcription services are provided in-house to ensure timely turnaround of patient results and enhanced patient care. The HIM department also employs electronic signature for the authentication of medical record reports and entries for all Weiss physicians.
The HIM department works closely with many other departments in the hospital. For example, HIM works with the administration department on billing and collections issues.
The HIM department is open from 8:00 a.m. to 4:00 p.m., Monday through Friday. After hours and on weekends, patients may contact the security department for questions related to medical records.
