Age-Friendly Health System – Committed to Care Excellence
Age-Friendly Health Systems is an initiative led by The John A. Hartford Foundation and the Institute for Healthcare Improvement in partnership with the American Hospital Association and the Catholic Health Association of the United States. It is a movement to deliver safe, reliable, high-quality health care in every setting based on what matters most to older adults as individuals (see ihi.org/AgeFriendly).
As part of the Age-Friendly Health Systems Action movement, we are among the first health systems in the country implementing age-friendly health care.

WISE, the Weiss Initiative Supporting Elders, is a free wellness program for adults age 60 and older.
